CNA FORMS
The following forms require signature of the parent and the student. You may sign the virtual signature form (see below in red) and submit to acknowledge all forms except Registration. The Registration form must be filled out and submitted separately.

Student/Parent Handbook
VIRTUAL SIGNATURE FORM FOR PARENTS - click on link below and hit send at the top to submit. Sending this form verifies you are in receipt of the forms listed above. The registration form needs to be filled out separately and submitted to the school.
The following forms require the signature of the student ONLY:
Criminal Background Disclosure Form 2018 (if student is younger than 17, parent signature is needed)
ADDITIONAL DOCUMENTS REQUIRED:
A photocopy of each student's drivers license or state ID
A photocopy of auto insurance card for each driver
A photocopy of each student's social security card
CNA FEES:
Required Fees:
Uniform top and pants purchased from Wilco: $40 per student due by Friday 8/31/18
AHA BLS CPR Certification with CPR Card: $25 per student due by Friday 8/31/18
Background Check Fee: $35 per student due by Friday 9/1/18
State Registration Fee: This fee will need to be paid in May 2019 during State CNA exam registration. Students will be instructed to the website where they can pay this fee and complete the process of registering for the state certification exam. Students will need a credit card or debit card to complete this transaction.
Optional Fees:
Gait Belt: $10 each
Class Text Book for personal purchase: $49 (new)
Workbook for class: $30 can be purchased on Amazon
HOSA Membership: $25
***Students are to pay fees in the Wilco Office and present receipt of payment to their instructor. All required fees must be paid as stated to prevent the student from being held back in clinical start.
